Notice on submitting the "Health Declaration Form Visa Application"

According to relevant laws and regulations of the People's Republic of China, starting from now on, all visa applicants (except diplomats) are required to submit the "Health Declaration Form for Visa Application" (attached), and the visa application will not be accepted if this form is not submitted.
